Our mission is to provide a full range of coordinated health and wellness services to qualified older adults in their homes, the PACE center, and the community so they may remain safely at home for as long as possible.

Innovative Care for Aging Individuals

Our goal is to provide our senior community members with the care, medical treatment and support they need to achieve the highest quality of life – while remaining independent for as long as possible. Our team of elder care experts offers an alternative to nursing home placement that features comprehensive, coordinated care for a senior’s medical, social and physical needs, while also providing peace of mind for family caregivers.

Adult Day Health Center

Our Adult Day Health Center is a vibrant space where seniors can receive support, enjoy meals, and socialize together. Our facility is focused on safety, independence, and building a community of seniors who desire to improve or maintain all aspects of their health.

Our next social worker to highlight for the month is Greg! Thank you for all you do and have done for our participants thus far!

I am Greg Burlager and I have been at PACE for about three months. I completed my bachelor’s and my master’s degree in social work from Saginaw Valley State University. My wife and I have identical twin 4 year old boys and a 7 month old baby girl. My family is where I spend most of my time. When I get time to myself I enjoy playing golf, playing video games, watching football, and engaging in the sport of wrestling, not the WWE, Star Wars, comics, and being outside. I have enjoyed the short amount of time I have been at PACE and I am grateful to work with our amazing staff and participants.

🏋️‍♀️Squats for February!🏋️‍♀️

Did you know that engaging in exercises- especially squats can assist with independent transfers when you get older?
28 PACE staff put in the work and completed over 10,000 squats during the month of February.
One staff member said “It was a fun challenge to get us up and moving throughout the workday. We kept one another accountable!” While another staff member commented “I didn’t realize how hard these squats would be, I need to increase my activity now so that I can be as independent as possible when I’m older.”
Participants also got a chance to join in on the squat challenge and helped add an additional 300 squats to the grand total!
